Additional fields in forum posts

I am looking for an extension to create additional fields in forum posts. I do not know how it is called correctly.
Example:
I open the topic on the forum registration and in the 1st post I prescribe a template from additional fields to fill out the questionnaire.
Further, all new user posts must be with the same fields of the questionnaire, and the user must fill them out.
Or advise something else, for registration forms.

All these extensions create a profile only in the 1st post.
But I need the same questionnaire fields to be in every new message from users.
