Author notes:
This is a port of the Ban Users After X Warnings MOD for phpBB 3.0.x by paul. This extension is functional, but may need to be adjusted if this pull request is merged into the core. Also, this is not an infractions system. If you are looking for an infractions extension, please see this Extension in Development.
