How often is it supposed to refresh? Based on forum/topic events ?

Its not needed to refresh because the sitemap is created dynamic. So every time Google fetch the sitemap it gets the current version with all forums/topics.

isn't that prone to timeout on large websites?

No, the extension generates a sitemap index with one sitemap for each forum and there it's a single SQL query, same as in the original forum too, only without pagination. So I don't think that you will get timeouts.

You can have 1,000 forums with 50,000 threads/pages in each forum. This are the limits for XML Sitemaps.
I don't think that someone have more than 1,000 forums in his board.
If you have more than 50,000 threads/pages in one forum (without subforums) you can't use this extension, but you have this problem also with a XML Sitemap generated by hand.

Does the sitemap only include public readable forums, or can you exclude the admin forum in the sitemap?

The sitemap uses the permissions system of the forum, so if you call it as admin you see the admin forums. If you call it as guest you see only forums that guests can see.

And if Google downloads it, it will see the forums accessible for bots then I guess.

Yes, if the bot that download the sitemap is detected as Google bot it see the sitemap with bot permissions.

If you have more than 50.000 threads in one forum may be its possible to split the content into 2 different sitemaps and add them to the sitemap index.

Not working with new version and can't load css from sitemap so looks ugly for human

Update to 0.1.3

Fixed the HTTP header output, so the style works again.
