This is the one thing that worked for my board. I use a security question as well, but humans can answer just about anything. Yeah, it delays their first post a little, but still better than having to clear out a bunch of spam constantly.

I added Wait for registration extension. Once they register they wait 10 secs then I add also spambot counter measures sortable captcha. Then I activated wait to approval in every post. Now the spammer stop. Hope it helps to other. :D
