New member registration without notification

New registrations have to fill in two required custom fields and must be approved by me (admin). I now have a registration that has not filled in the required fields and I have not received a notification. How could that be possible (I'm using 3.2.0)?

Are you sure it's a registered account and not a guest post?
Can you see the account activation in the admin log?
Are you sure the account didn't register before the registration conditions were set up?
Do you have any external integrated registration?
Are you sure you haven't got tapatalk from an old installation still running?

I will post some pictures later to clarify. There is no guest post as I haven't approved the registration. The registration conditions haven't changed.

Basically it's not possible to bypass the standard registration. There should be a logical reason for what you're seeing. Either a setting isn't what you think it is or the account is registering/posting legitimately by another means on the site.

When I try to register I got the following errors
You have provided an invalid answer to the question.
The field “Bikes” must be completed.
The field “Real name” must be completed.
So no way to skip that.

I know, but as you can see it did happen. There is also no notification record, so it's very weird.

hmm.
That Mikeot account isn't actually active. It maybe looks like you just didn't get the notification, which could be a server blip.
