Have a read of the last paragraph of Knowledge Base - How to configure Q&A CAPTCHA.

There are no ‘threads’ in phpBB, they are topics.

You can use the Q&A CAPTCHA as a registration code.

Q: Type in the Registration code.

A: the registration code you have provided

Then, email the registration code (any string of numbers and/or letters) to the prospective registrants.

How to configure Q&A
  1. Administration Control Panel (ACP) > Spambot countermeasures> Installed plugins > click the dropdown box and select Q&A (it will be greyed out)
  2. Click the configure button then click add
  3. Add your question and answer (you need a Q&A for each language you have installed) click submit
  4. Click "back to previous page" then click "back"
  5. Select Q&A from the dropdown box (again)
  6. Click submit at the bottom of the page.
  7. Done.

There is also a setting that allows 'Admins' of the site to be able to Approve or Disapprove a registration attempt.

ACP > User registration settings (left hand side) Account activation > By admin (Topmost Option).

Use of this would allow you to 'vet' prospective members before allowing them access to the site.

Yeah, 500 times. Or more, because there will be nonapprovals. Not a good idea.

Think I should have made that a bit clearer.
I did not mean on it's own, I meant in addition to the Q&A and other measures used to prevent Spam registrations.
I use the 'Sortablescaptcha' from Derky as well as the Account activation by Admin on a moderately large busy site and only get a few activation requests.
The most I have seen so far is three and often there is none on any particular day.

The OP mentioned only allowing from certain email addresses and the Account activation by Admin shows those so thought it would be a useful addition for the OP.

If he does it as I said, there will be no need for account activation. No one without the code will be able to register.
