ppf, disable all extensions, see if it still happens. If not, enable them 1 by 1 , and test.
1. I've had another look at the logs and there doesn't appear to be any more information. There is a second error which is exactly the same but for a slightly different file

The site was originally migrated across from phpBB2.0 so I wondered if something had become corrupted during the migration, causing the circular referral, but I don't know enough about it to be able to guess what it might be.
2. I did find that I'd missed the final / off the RewriteBase directory in the .htaccess file and have added that in in case it makes a difference.
3. I disabled all extensions but it makes absolutely no difference with the notifications problem. I can't see if there is still an apache log error because the logs are only updated on the server once per day, so I can't access them till tomorrow.
