Most of the extensions I've created aren't even released here because they are specific to my board. So in reality, your question is best answered by you. What do your users want? What functionality in their opinion is needed to make their experience pleasurable? Those questions should dictate what extensions you install.

I've been using phpBB for 17 years now and have never needed or used any mods or extensions.

Everything runs just fine for me without them.
