no, they are not all at fault unless you want to consider not reading the instructions about which is the proper update method to use.
the auto update does work depending on the situation but since extensions have replaced the need for editing files there is rarely a need for the auto update any longer and the file replacement method is much quicker and easier than messing with the auto update.

In my opinion the auto update should have been dropped when phpBB 3.1 was released.

I knew that. The point was it changed functionality for members but wasn't highlighted in the release announcement.
