Template changes aren't the problem, it's the underlying code passing the data.

So your idea is that i try to adapt a mod without knowledge of coding... Forget about phpBB 3.0 and focus on the systems currently in use.
The 3.0 mod is a simple one (I actually extended it for my board at the time to include hermaphrodites) and the same can be said about the Gender extension. You could almost take it as it is and adapt it to your needs (in steps) - perfect to start with.
Your opening post does specify your intention to "make" something.
Which mod?
Much has changed with both CPFs and template files between 3.0 and 3.2 so it may or may not be possible but I suspect that what you are wanting to do will require an extension
