I'm pretty sure it will work on 3.0.4, but if your still unsure just make a copy of your forum in a different directory on your server and test it that way.

You've installed it wrong, you must of missed some code. If you leave out or delete just a > off the end of some code you will get a blank page. Re-check everything, you should find something wrong...correct it. If not, apply your backup files.

Is there a way to make this mod appear to everyone instead of users having to turn it on through User Control Panel?

It should do that by default. Did you install it correctly?

Warning: include() [function.include]: Failed opening './common.php' for inclusion (include_path='.;C:\php5\pear') in C:\wamp\www\forum\contrib\install\install.php on line 17

I'm sorry men, I was away for 3 weeks and didn't have any PC under my hands.
If your issues are still actual, please feel free to remind it again.

Note: install.php is to be running from the phpbb root directiory without any subfolders.

And since rxu.phpbbguru.net is down, I'm attaching my fixed version of install.php here.
