I am upgrading from 3.3.11 to 3.3.14.

Do I need to upgrade language packs? (American English is the only one I have)
Do I need to do anything for the extensions?

If the British English language pack differs from that of your initial version, then the other language pack should also be updated in order to avoid untranslated (or old incorrect) translations being shown on your board.

Not necessarily, it depends on whether the extension remains compatible with the new phpBB version or not.( PHP version and/or phpBB code compatibility)
That's something you should check before updating.
Added: use a file/directory compare program to compare the language/en folder of your previous phpBB version to the language/en of the new version. That way you'll easily find the files of the US english language pack that need edits.
Spelling is freeware, which means you can use it for free.
On the other hand, it is not open source, which means you cannot change it or publish it in a modified form.

Updating language files takes just some minutes to do, so why not. Sometimes they add new entries to the language file and if you update the platform and use the older lang pack, you will miss those entries.
For the default phpBB language you can find all the files changed between phpBB versions in the Changed Files package
