I use it on two of my forums and is working fine for me. Just try installing it, you can always uninstall it easily if you need.

Where do I place the Moderator_ban_user file?

You don't as there is no such file.

You click the download link and download the mod which will be a set of instructions and a single file called ban.php
You open the install instructions in a text editor such as Wordpad and follow the instructions to the letter.
It involves downloading two files and then editing them according to the given instructions. They are then ftp'd back to the server along with the ban.php.
ban.php goes into the root of your board alongside config.php
