check to make sure you have the files
/includes/ucp_mypage.php
/includes/info/ucp_mypage.php

Are you using the newest phpbb3?
Are you sure you added the permissions for the correct user/group?

I did check those files, they are there... I did assure permissions are set... I'm using 3.0.4 PHPBB. Still trouble setting it up...

Should have added 9 total modules, 4 ACP modules and 5 UCP modules.

I have 5 UCP modules, and 3 ACP modules (Overview, Limit Options, and Style options)

I did a complete reinstall of all my mods when the latest phpbb update arrived and suddenly your mod worked like a charm

Still need help please? The only factor I can figure is my prefix for my sql database is not PHPBB_ The sql tables seem ok, but is there some code in the uploaded mypage files that are looking for the prefix PHPBB_?

Problem Solved thanks to Josh! Josh found I had too many modules installed in my sql database for an unknown reason. With his direction I deleted all the modules and then ran the install program, and works like a charm now. Very, Very cool mod... Thanks Much to Josh...
