[REQ]Post Dynamic URL into Static URL with the Post Section

Okay..
To the topic..
Example :

I reply in topic with this subject : "Re: Support Us"
with this for the post url : http://localhost/viewtopic.php?p=13
I want it change into : http://localhost/re-support-us-p13.html
Remember this is post not topic..

Can someone Help me ??
I'm stuck for editing the SEO MOD >.<

SEO mod only changes the post into : postxx.html..
Example www.example.com/post125.html
I want to change it into : www.example.com/just-example-post-subject-p125.html
