Go directly to Erik Frèrejean's post for the (still) current answer.
Take note that the answer is directly related to the downloadable or not status of the SRT code, and that the next post gives a strong hint on how to get a similar SRT (which will need some serious tweaking in order to allow use on personal sites).

There is a fork of STK that works on modern versions of phpBB: https://forums.caforum.fr/viewtopic.php?t=41
It has the code for the Support Request Template inside it which should work on newer phpBB versions. In order to allow users to use it, you will have to find that code and use it to create your own program though.
