Can you please tell me which phpBB version you are using? Are there any other extensions installed? Does it work having all other extensions disabled? Does the “Delete all cookies” link in your forum work correctly? Can you switch accounts having the AJAX capabilities on the settings page?

There’s an option to disable AJAX in the extension’s settings page under the Extensions tab. Once you disable, try doing it again.
