What about after chosing country, appear State options and after State, appear city options?

Uhmmm, yeah that is quite an impossible request. EG, for the US there are 50 states and ~20k cities...just for the US and I'm pretty sure not each city has a flag or where one would even get them.

No flags for states neither cities. There are WordPress plugins with this kind of database.
