I have noticed that when one image is inside a spolier, it is converted to a thumbnail, and there's no way to see it in it's regular size

Is there any way that this ext doesn't affect to spoilers?

Its going to be a conflict with the spoiler code you are using.

This is the spoiler I use and its working perfectly with my quoted image as thumbnail ext.

Perhaps you could change your spoiler code? If not let me have a link to the one you use so i can do further testing.
