This is weird. I am doing a clean install using latest phpBB version and PHP 7.2.

I extracted the zip file and put the folder, mentions, in the /ext/phpbb/ directory but I'm not seeing it listed on the ACP Extension list and am unable to activate it.

I installed another extension to test, mediaembed, and had no problems.

I even disabled both to double check. No luck.

Any ideas?

you did put it in the wrong directory.

In ext/phpbb/ belong only official phpBB-extensions. Simple mentions is not one of them.

So basically every extension needs to be in this place "ext/<author>/<extensionname>" => "ext/paul999/mention/". If you are unsecure about this, take a look in the composer.json and there into "name": "paul999/mention",
