How to remove old ACP_BLOCK_LOGS menu item

I think I installed Advanced Block MOD on my forum years ago and at some point uninstalled it. Or mostly did.

Now when I go to my admin's Maintenance tab, there's a menu item on the left reading "ACP_BLOCK_LOGS".

When I click it I get a page with a title of "TITLE" and the top of the page has these errors:
[phpBB Debug] PHP Warning: in file [ROOT]/includes/acp/acp_logs.php on line 48: constant(): Couldn't find constant LOG_BLOCK
[phpBB Debug] PHP Warning: in file [ROOT]/includes/functions_acp.php on line 137: Cannot modify header information - headers already sent by (output started at [ROOT]/includes/functions.php:3268)
[phpBB Debug] PHP Warning: in file [ROOT]/includes/functions_acp.php on line 137: Cannot modify header information - headers already sent by (output started at [ROOT]/includes/functions.php:3268)
[phpBB Debug] PHP Warning: in file [ROOT]/includes/functions_acp.php on line 137: Cannot modify header information - headers already sent by (output started at [ROOT]/includes/functions.php:3268)
I guess I didn't completely uninstall it, but I'm stumped as to what remaining bits I need to tidy up. Any ideas?
