What would you charge to upgrade this or release the source code so I can get someone to upgrade this for me?

It's not allowed to offer payment in any form for assistance in this forum. If you feel what you want to do is beyond your capability, then please post in the Wanted! forum where you may find someone willing to take on your project.

Also I doubt the author will be willing to do this as s/he no longer supports this. I suggest you post in Extension Requests or Wanted with details of what you are looking for

Very little, actually! I have finally managed to release a Forms extension which isn't exactly port of the MOD, but serves the same (but also more wide) purpose. It took me a lot of time to make so I decided to ask for a small fee for it. I won't open-source it, but that allows me to offer it for a very low price to everyone.

Of course, you can also see the demo of Forms extension before you purchase.
